A 48 in the 15th took O'Brien to within a frame of victory at 9-6 up but with the finishing line in sight, O'Brien's potting fell apart under pressure.
Hawkins took the next three, with a 129 in the 16th, but missed a black in the 19th allowing O'Brien to seal victory.
Victorious Fergal O'Brien said the emotions felt during the deciding frame caused him to punch the air in delight.
